SUBCONTRACTOR EXCAVATION CHECKLIST <br /> GENERAL INSPECTION OF JOBSITE <br /> Competent Person: <br /> Sal Type: <br /> Excavation Depth: <br /> Excavation Width: <br /> Protective System: Sloping, Shoring,Trench Shield <br /> Daily Trenching Log: <br /> UTILITIES REVIEW <br /> Exact Location of Utilities marked. <br /> Underground installations protected. <br /> MEANS OF ACCESS AND EGRESS <br /> Ladders secured and extend 3 feet above the edge of trench. <br /> Structural ramps designed by a competent person. <br /> Employees protected from cave-ins when entering or exiting trench. <br /> WET CONDITIONS <br /> Precautions taken to protect employees from the accumulation of water. <br /> Water removal equipment monitored by a competent person. <br /> Surface water or runoff div <br /> Inspections made after every rainstorm. <br /> HAZARDOUS ATMOSPHERE. <br /> Obtain a Confined Space Permit from LLNL. <br /> Atmosphere within excavation tested for Oxygen deficiency, combustible or harmful <br /> contaminant. <br /> Ventilation provided to prevent employee exposure to flammable gases. <br /> Testing conducted regularly to ensure a safe atmosphere. <br /> Emergency equipment available for rescue. <br /> Employees trained to use personal protective and rescue equipment. <br /> Safety harness and lifeline used and individually attended when entering bell bottom or <br /> deep confined excavations. <br /> SUPPORT SYSTEMS <br /> Materials and equipment used for protective systems inspected and in good condition. <br /> Damaged materials and equipment used for protective systems inspected by a registered <br /> professional engineer after repairs and before being placed back into service. <br /> Protective systems installed without exposing employees to hazards of cave-ins. <br /> Support systems provided to ensure stability of adjacent structures,buildings,roadways, <br /> sidewalks,walls,etc.. <br /> Excavations below the level of the base or footing supported,approved by an RPE. <br /> Shield system placed to prevent lateral movement. <br /> Employees are prohibited from remaining in shield system during vertical movement. <br /> i" <br />
