CLOSURE REPORT FOR UNDERGROUND STORAGE TANK 805-DlU1 <br /> PERSONAL PROTECTION EQUIPMENT(PPE)DISPOSAL: <br /> [X] Not Applicable. (Used PPE did not require special handling or disposal) <br /> The PPE was transported by: N/A <br /> The PPE was disposed of as: N/A. Date: N/A <br /> Name and address of disposal facility: N/A. <br /> [N/A] LLNL Hazardous Waste Requisition and Uniform Hazardous Waste Manifest"TSD <br /> Facility to Generator"copy for the used PPE enclosed. <br /> CLOSURE REPORT CONCLUSIONS: <br /> UST 805-DlU1 was closed according to the Closure Plan approved by the San Joaquin County <br /> Public Health Services. <br /> • On June 22. 1994, the tank and supply/return ping along with the vent line piping were <br /> removed by PetroTek and transported by Erickson to the Erickson facility. Samples were <br /> collected from within the tank excavation(at a depth of 11 feet)and from the soil stockpile. The <br /> results of the TPH-D analyses for sample TK-01-805-01-D1U1-SO-SP (collected from the <br /> excavated soil stockpile) indicated 7.4 mg/kg of diesel fuel, and BTEX constituents were not <br /> detected. The soil sample collected from within the excavation (11-foot depth), indicated no <br /> TPH-D or BTXE compounds. <br /> • The stockpile soil from the UST 805-DlU1 excavation has not been remediated, nor has a <br /> method of proper disposal been determined. The concrete tank anchoring structure will be <br /> properly disposed of upon the determination of disposal method for the stockpile soil. <br /> • At this time.LLNL does not plan any further action for this tank closure. <br /> [X] Photographs of tank closure activities are enclosed as Appendix D. <br /> [N/A] A copy of the State Water Resources Control Board UST Permit Application Form B <br /> shall be submitted to your office within 30 days of the submittal of this report. <br /> [N/A] A copy of the State Water Resources Control Board UST Permit Application Form B <br /> was submitted to your office with the UST Closure Plan. <br /> OTHER OBSERVATIONS: <br /> • An Underground Storage Tank Unauthorized Release(Leak)/Contamination Report was sent <br /> to Mr.Doug Wilson of the Public Health Services of San Joaquin County on July 11. 1994 (see <br /> Appendix F)based on the resulting analyses of soil samples collected on June 22,1994. <br /> • Tank testing had shown this tank to be leak tight. <br /> • Weather was sunny and windy. <br /> • Soil compaction was performed as indicated on the Consolidated Engineering Labs Field <br /> Density Report(see Appendix G). <br /> • The certificate of tank destruction is enclosed as Appendix H. <br /> 5 <br />
