Minimum Personal Prntp�a�.. <br /> SLRfma►\/ <br /> Identify minimum level of personal protective equipment required for each of the <br /> identified phases of the project. <br /> Indicate specific type of protective clothing/equipment required: <br /> coveralls - indicate fabric type (cloth, Tyvek®, polyethylene coated Tyveka, etc.) <br /> gloves - indicate material and weight (heavyweight neoprene, medium-weight <br /> nitrile, viton, heavyweight latex, PVC exam-type, leather, etc.). <br /> respirators - indicate facial coverage (half, full, hood) and air <br /> source/purification element (SOBA, organic vapor cartridges, NEPA cartridges, <br /> Type N cannisters, etc.) <br /> eye protection - indicate facial coverage (safety glasses with sideshields, <br /> chemical-splash resistant goggles, faceshields, etc.) <br /> • In "Other" section, specify additional equipment/clothing including, but not limited to <br /> hearing protection (expanding foam plugs, circumaural muffs, etc.) aprons, and <br /> metatarsal guards. <br /> Work / Task Hazard 4nalYsis Summary <br /> - Duplication of sheet may be necessary. <br /> • List individual tasks performed at site. For-each task, identify all <br /> safety hazards, including, but not limited to fires, explosion, physical <br /> Possible health and <br /> (crushing, excavation cave-in, falls, etc.), chemical agent exposures (inhalation nr and <br /> dermal), and physical agents (noise, radiation, etc.).
