NARRATIVE OF OBSERVATIONS/DISCUSSION WITH OPERATOR
August 16, 2001
Upon arrival at LLNL Site 300, 1 was joined by Alan Jessen, Industrial Hygienist, assigned to the Berkeley
Office. We obtained clearance and were provided security badges with dosimeters. While the LLNL Site 300
security personnel processed our badges, Alan Jessen was shown a site safety film. After the security badges
were issued, we went on to Bldg. 873 where we met with Dan Knight.
An in-briefing was held. Dan Knight (Dan), Dawn Chase (Dawn), Stan Terusaki (Stan), Rob Tageson (Rob),
Alan and myself were the attendees for this meeting. I began the in-briefing by explaining the reason for the
visit. I then explained that the inspection would consist of walk through observations of permitted and
generator areas. In addition, I would interview employees, take photographs if necessary, review and copying
documents. I then asked for consent to conduct my inspection. Dan gave consent for the inspection.
At this point I turned the meeting over to Alan. Alan explain his role and what he would be doing during this
inspection. I added that we would visit the same areas that had been visited during the previous inspection, in
DTSC (3/95) Page 5 of 11
