Box 1025 <br /> West Sacramento, CA 95691 <br /> Re: CRLLC#2705446, 1403 Country Club Blvd, Stockton, CA SR0059964 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to repair a Bravo float and chain on dispenser 1 and replace the 87 <br /> product spill container on the underground storage tank (UST) system at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda :supersede the previous ones. <br /> 3. Contact Garrett Backus at(209) 468-2986 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test(or equivalent). Proper drainage must also be verified. <br /> c. Verify functionality of the dispenser 1 Bravo float and chain. <br /> Please be aware that the facility cannot receive fuel in the 87 tank prior to the EHD <br /> inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically presentat the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br />
