Box 507 <br /> Vacaville, CA 94533 <br /> Re: United Pacific 5446 SR0077933 <br /> 1403 Country Club Blvd. <br /> Stockton, CA 95201 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to replace a Bravo UDC float and chain in dispenser%- 91 product, and <br /> replace bellows to the waste oil spill container on the underground storage tank (UST) system at the <br /> above noted address. In addition to the noted replacements, the diesel annular sensor model Veeder <br /> root 303 and the Ronan LS3 sensor in the waste oil fill sump were replaced at, or around May 25, <br /> 2016. These components were tested during the annual monitoring system certification, therefore do <br /> not need to be retested as part of the permit conditions. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aaron Hang at (209) 468-3453 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). <br /> a. Spill bucket test for the waste oil spill container: Verify minimum 5-gallon capacity and pass <br /> a one hour hydrostatic test (or equivalent). Proper drainage must also be verified. <br /> b. Demonstrate functionality of the float and chain for dispenser%- 91 product. <br /> Please be aware that the facility cannot operate the UST system tank prior to the EHD <br /> inspection and approval. <br /> 4. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 1868 E.
