None of the oil change facility employees have been trained by the designated operator within the <br />past 12 months. The designated operator shall train facility employees for which he or she is <br />responsible in the proper operation and maintenance of the UST system once every 12 months. The <br />training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees, including the oil change facility employees, have been trained by the designated <br />operator, maintain the list on site, and submit a copy of the training records to the EHD by June 20, <br />2013. <br />305. The dispenser 1/2 east float and chain failed to stop the flow of product at the dispenser when <br />tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br />flow of product at the dispenser when it detects a leak. The technician adjusted the chain to repair the <br />device and a retest passed. Violation resolved. <br />305. The dispenser 1/2 diesel product shear/impact valve did not close when tested. All monitoring <br />equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br />dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, and <br />certified contractor replace the failed component with a functional component (LG 113-x listed, if <br />applicable) and obtain a permit within one business day from the EHD.
