Any release from the primary containment <br />that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be <br />recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br />containment <br />Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br />records. Submit a copy of the recordable release statement to the EHD. <br />This is a Class II violation. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />There was an 1-2: 87 STP fuel alarm on October 31,2016. The DO report refrenced work order 91962 on the DO <br />inspection report, however, this work order does not address the fuel alarm. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />This is a Class II violation. <br />319 <br />CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The spill container for the used oil UST failed the hydrostatic test. Immediately contact a properly licensed, trained, <br />and certified contractor to install an approved spill container under permit and inspection of the EHD. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />This report is an addendum to the inspection report dated on 5/1/17. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 6/18/17 . <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br />Page 4 of 5 <br />
