Ms. Le questioned <br /> Ms. Webb because the earlier report from Elite IV stated that everything was functional.Ms. Webb did not <br /> comment. <br /> 12/16/04 Ms. Le went to the facility to witness the monitoring certification.Before beginning the monitoring <br /> certification the alarm history was printed from the VeederRoot monitoring panel by Ernie,the Elite IV <br /> technician. The alarm history showed that only 3 out of the 8 sensors were tested on November 4, 2004. <br /> Ms. Le spoke with Ernie and he said he did not know why the other alarms did not print. Ms. Le asked Ernie if <br /> their protocol is to test all the sensors including the annular during the pre-test and he said,"absolutely". <br /> Ms. Le obtained a copy of the work order from the facility.The work order states the following: <br /> Work ordered: pre-inspection for monitoring system certification <br /> Description of work done:Tested all tank fill sensors at 91,87-,X2.All tank fill sensors are working correctly. Tested all STP <br /> sump sensors all working and shutting off STP pumps. Ran 3.0 test at both 87,91 leak detectors both working good.All site <br /> sensors and probes for monitoring system working in good order. <br /> This underground storage tank system is comprised of 3 tanks, 1-9loctane 2-87 octane(master&slave). This <br /> underground storage tank system is not equipped with fill sump sensors.The only sensors that showed alarm on <br /> November 4,2004 were the 3 STP sump sensors. None of the sensors were tagged with the required contractor <br /> information from the pre-test.
