Fees for the annual permit to operate the UST system have not been paid. No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. Immediately contact the EHD accounting office to make arrangements for payment to obtain a permit to operate a UST system. This is a Class II violation. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and/or not approved by the EHD. The monitoring plans for all three tanks list both FE Petro and Red Jacket leak detectors, rather than the specific leak detector for each tank. The comment in the monitoring plans regarding the vapor pot sensor is incomplete and stops midsentence. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class 11. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The Monitoring Site Plan does not include the locations of the monitoring panel or the UDC sensors. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 115 CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. A new designated operator was hired and notification was not provided to the EHD within 30 days of the change. The following designated operators performed monthly inspections at the facility and are not listed on the Owner Statement of Designated Operator Compliance submitted to the EHD: Ron Rocha - June 27, 2014 inspection Ruben Galindo - July 16, 2014 inspection Jose Ochoa - February 24, 215 inspection Jacob Weaver - March 23, 2015 and April 27, 2015 inspections Any changes shall be reported to the EHD within 30 days.
