The following test dates were incorrect: <br />-The SB989 test date was filled in with a question mark instead of a date on the February, March, and April 2015 <br />reports. <br />-The Monitoring System Certification, Spill Bucket testing, and Leak Detector testing were incorrectly dated 3/31/15 <br />on the April 2015 report. <br />-The Monitoring System Certification and Spill Bucket testing were incorrectly dated 5/30/13 on the June 2014 report. <br />The annual Monitoring System Certification was last conducted on May 29, 2014. The SB989 secondary containment <br />testing was last conducted on May 16, 2012. Both tests were completed today. During the monthly inspection, the <br />designated operator shall check that all required testing and maintenance for the UST system have been completed <br />and document the dates they were done. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />209 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />No current designated operator training records were available on site. The most recent training found on site was <br />dated 2013. The designated operator shall train facility employees for which he or she is responsible in the proper <br />operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a minor violation. <br />309 <br />CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br />The Bravo box float and chain in the dispenser 5/6 (north) UDC sump failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak.
