C <br />11 <br />The following is an itemized list of underground storage tank violations that have not <br />been addressed for AM PM HAMMER/15 FOOD #83113 as of January 02, 2015. <br />Open violations from May 29, 2014 inspection <br />Violation #104 - UST Operating Permit Application for Facility and Tank information not submitted or <br />current. <br />UST Facility Operating Permit Application and UST Tank Information have not been submitted in CERS. Any change <br />of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the <br />required information, and submit for review by the EHD. Complete a separate tank form for each tank. This is a Cla <br />II violation. <br />Violation #106 - Monitoring and response plans not current or approved by the EHD. <br />The monitoring plans have not been submitted to CERS and are not approved by the EHD. The monitoring and <br />response plans must be current and approved by the EHD. Immediately log into CERS. complete the required formE <br />and submit for review by the EHD. Maintain an approved copy on site. This is a repeat violation, Class II. <br />Violation #115 - Failed to inform the EHD of a change of designated operator within 30 days. <br />A new designated operator, Josh Brown, conducted the monthly inspections for November 2013, December 2013, <br />January 2014, February 2014, and March 2014, and notification was not provided to the EHD within 30 days of the <br />change. A new designated operator, Ron Rocha, conducted the monthly inspections for April 2014 and May 2014, <br />beginning April 15, 2014, and notification was not provided to the EHD within 30 days of the change. Any changes <br />shall be reported to the EHD with 30 days. Immediately, submit notification to the EHD identifying all the designated <br />operators for this facility. Alternatively, log into CERS, upload the required information, and submit for review by the <br />EHD. This is a repeat violation, Class II. <br />Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance records for the last three years were not found on site. Maintenance and monitoring records shall be <br />maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling <br />(2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of <br />sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, <br />ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locat <br />and maintain all maintenance records for the last three years on site and submit copies to the EHD. This is a minor <br />violation. <br />Violation #204 - Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not found on site. A copy of these plans shall be retained on site at <br />times. Immediately locate and retain a copy on site. This is a repeat violation, Class II. <br />Violation #208 - Designated operator monthly inspection reports not maintained on site for at least 12 <br />months. <br />The August 2013 designated operator monthly inspection report was not found on site. Designated operator month <br />inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the <br />previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to <br />the EHD. This is a repeat violation, Class II. <br />Violation #320 - Failed to maintain all sumps (including UDCs) free of debris and liquid. <br />Liquid was observed in the 87 master fill sump. If water could enter into the secondary containment by precipitation c <br />infiltration, it must be removed and disposed of properly. The service technician removed the liquid from the 87 mas <br />fill sump. Ensure that all sumps and annular spaces are maintained free of liquid. This is a minor violation. <br />Page 1 of 1 <br />
