Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Department (EHD). Current financial responsibility documents are required to be <br />submitted annually. Received during the inspection - violation resolved. <br />106. Current approved copies of the monitoring plan are not in the EHD file. The vapor pot sensors <br />and pump out procedure needs to be added to the monitoring plan. The monitoring plan must be current <br />and approved by the EHD. The updated monitoring plan was approved during the inspection and a copy <br />was made for the EHD file. Violation resolved. <br />108. Secondary containment testing was performed on May 16, 2012 and the test report was <br />submitted to the EHD on December 11, 2012, several months late. A copy of the test report must be <br />submitted within 30 days of the test. Violation resolved. <br />113. A new designated operator Lyle Meeks was hired in February 2013 and notification was not <br />provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 <br />days. Notification was received in writing during the inspection. Violation resolved. <br />313. The 87 master tank spill container failed when tested. All spill containers shall have a minimum <br />capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br />Immediately discontinue deposition of petroleum into this tank until the component is repaired or <br />replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br />If the spill container can't be replaced immediately, there is a possibility that the 87 octane UST system <br />may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
