Secondary containment testing was last performed on 4-21-09 and was due before May 2012. <br />Testing has not been completed and is one month past due. Secondary containment testing is required <br />once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. <br />113. A new designated operator, Edward Deyoung was hired on 8-29-11 and 9-20-11 and notification <br />was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD <br />with 30 days. Submit notification to the EHD identifying all the designated operators for this facility by <br />6-1-12. <br />204. The designated operator failed to check and note the correct test dates for the secondary <br />containment test on the August 2011 through January 2012 designated operator monthly inspection <br />reports. The secondary containment test was listed as 4-2-09. It should have been listed as 4-21-09. <br />During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. , <br />205. The February 2012 designated operator monthly inspection report was not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br />reports are maintained on site. Submit copies to the EHD by 6-1-12. <br />305. The Bravo box float and chain in the sump failed to stop the flow of the 87 product at the #8 <br />dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br />alarm or stop the flow of product at the dispenser when it detects a leak.
