(Padre), on behalf of the Petro-Analytical Inc. (PAI) and Kaiser <br /> Permanente (Kaiser)submitted the document titled Technical Work Plan/Site Health and Safety <br /> Plan, Limited Soil and Groundwater Assessment Activities at the Kaiser Permanente Manteca <br /> Medical Center, 1777 West Yosemite Avenue, Manteca, San Joaquin County, California and <br /> dated May 2, 2011. The work plan outlined proposed tasks to safely complete soil and <br /> groundwater assessment activities related to the in-place closure of one, 8,000-gallon diesel-fuel <br /> underground storage tank(UST). <br /> The subject addendum has been prepared in response to the San Joaquin County <br /> Environmental Health Department's (SJCEHD) request for additional drill holes to assess <br /> potential subsurface impacts related to the fuel oil supply (FOS) and return (FOR) underground <br /> piping. After removing a section of concrete at two locations, the FOS/FOR piping will be <br /> exposed and drill holes DH-3 and DH-4 will be advanced adjacent to the piping. Refer to the <br /> attached Plate 3- Drill Hole Locations. The drill holes will be manually advanced using a hand <br /> auger to an approximate depth of 1-foot beneath the piping, and soil samples will be collected <br /> using a slide hammer fitted with a stainless steel corer. Soil samples will be collected in stainless <br /> steel sample sleeves, sealed with Teflon tape, affixed with plastic end caps, labeled, then placed <br /> and preserved in a chilled cooler. The soil samples will be chemically analyzed for the presence <br /> of total petroleum hydrocarbons (TPH) identified as diesel fuel (TPHd) using U.S. EPA method <br /> 8015, modified, and benzene, toluene, ethylbenzene, and total xylenes (BTEX) and methyl-tert <br /> butyl ether(MTBE) by EPA method 8260B.
