(Padre), on behalf of the Petro-Analytical Inc. (PAI), is <br /> presenting additional information related to the permit package submitted for the in-place <br /> closure and abandonment of one, 8,000-gallon diesel-fuel underground storage tank <br /> (UST) at the Kaiser Manteca Medical Center, 1777 West Yosemite Avenue, Manteca, <br /> San Joaquin County, California(Project Site). <br /> This information has been prepared based on the San Joaquin County <br /> Environmental Health Department (SJCEHD) letter dated November 30, 2010 <br /> requesting clarification and additional information. The specific items outlined in the <br /> SJCEHD letter(attached)are addressed below and the revised permit application pages <br /> are attached: <br /> 1. Uniwaste will be responsible for off-hauling UST residuals (Section 6). <br /> Uniwaste is located at 17100 Lambert Road in Ione, California 95640. <br /> Uniwaste's hauler registration number is 4919. <br /> 2. Although Clearwater will be utilized to decontaminate the UST, Uniwaste <br /> located at 17100 Lambert Road in Ione, California 95640 will transport the <br /> rinseate material. Uniwaste's hauler registration number is 4919. <br /> 3. The answer to Section 14 of the permit application is 'Yes'. Padre <br /> understands that the 8,000-gallon UST will be replaced with an 8,000-gallon <br /> aboveground storage tank(AST). <br /> 4. Based on the proximity of underground utilities over and in close proximity to <br /> the UST (letter from Mr. Aaron Newman, P.E. -- Culpepper + Forgensi <br /> Architects)and the restricted work space around the UST, Padre proposes to <br /> advance two vertical drill holes as near to the UST as can be safely <br /> accessed. A copy of Mr. Newman's letter is attached. The drill hole locations <br /> are proposed along the east side (DH-2)and north side (DH-1)of the UST as <br /> shown on Plate 3- Drill Hole Locations presented in Padre's Technical <br /> Workplan and Health and Safety Plan. However, the as-built locations will be <br /> determined in the field based on the location of underground utilities and <br /> structures.
