-- SITE SAFETY PLAN
CLIENT NAME%S►►
COMPANY NAME: nc2
PROJECT ADDRESS:
PROJECT NUMBER: (3
A Site Safety Plan(SSP) has been designed to address safety provisions during the project. Its Purpose is to
provide established procedures to protect all on-site personnel from direct skin contact, inhalation, or ingestion
of potentially hazardous materials that may be encountered at the site. The SSP establishes personnel
responsibilities, personal protective equipment standards, decontamination procedure, and emergency action
plans.
All personnel participating in the field must be trained in the general and specific hazards. All site personnel,
project manager and/or site safety officer may impose any other procedures that they believe are necessary for
safe operations.
This plan is prepared to inform all field personnel, contractors, and subcontractors of the potential hazards on
the site. However, each contractor or subcontract must assume direct responsibility for his own employees'
health and safety. ^\
PLAN PREPARED BYom: "-
PLAN APPROVED BY:
PLAN REVISED BY:
POSSIBLE HAZARDS ON THE JOB:
2. 7. 12.
8. 13.
4. 9. 14.
5. 10. 15.
REQUIRED PERSONAL PROTECTIVE EQUIPMENT FOR THE PROJECT:
1. 5. 9.
2. . 6. 10.
3. -m-.S 7. 11.
4. 8. 12.
