A completed "Application for UST Closure Permit". A copy of the application is enclosed. <br /> 2. A written approval from the local fire department permitting the closure in place. <br /> 3. All items required on the "Closure in Place" application. This includes a technical work <br /> plan signed by a registered geologist or a qualified consultant and a completed "Well & <br /> Boring Permit Application" with the $122 fee for the soil borings. A "Well & Boring Permit <br /> Application" is enclosed. <br /> 4.
