CLASS 11 or WNOR-Notice to Com <br />2. Secondary containment testing was last performed on 10/21/09 after a piping repair and was <br />completed on 11/5/10, 7 months past due. Secondary containment testing is required six months aftei <br />installation and once every 36 months. Take all necessary precautions to ensure testing is performed in <br />a timely manner. <br />3. UST tank forms on file with the EHD are not current. Any change of information on these forms must <br />be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the <br />facility and tank forms to the EHD. Complete a separate tank form for each tank. The tank form was <br />updated while on site and a copy was submitted to the EHD. <br />40. Facility does not perform daily checks of the monitoring system and no leak detector is installed or <br />the diesel tank.
