Secondary containment testing was last performed on 10/21/09 after a piping repair and was <br />completed on 11/5/10, 7 months past due. Secondary containment testing is required six months after <br />installation and once every 36 months. Take all necessary precautions to ensure testing is performed in <br />a timely manner. <br />3. UST tank forms on file with the EHD are not current. Any change of information on these forms must <br />be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the <br />facility and tank forms to the EHD. Complete a separate tank form for each tank. The tank form was <br />updated while on site and a copy was submitted to the EHD. <br />40. Facility does not perform daily checks of the monitoring system and no leak detector is installed on <br />the diesel tank. For an emergency generator tank system, the leak detector requirement is satisfied if <br />the continuous monitoring system activates an audible and visual alarm in the event of a leak or <br />malfunction of the monitoring system, provided that the monitoring system is checked at least daily by <br />remote electronic access or on site visual inspections, and a log is kept of these inspections. <br />Immediately begin daily inspections and maintain a log of the inspections, or install a line leak detector. <br />Notes <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, within 30 days of receiving this report. <br />Provided free underground storage tank and hazardous waste class information. <br />A line test was performed during the monitoring certification and EHD was not notified. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector:rr <br />/V4/%� <br />Received By: //�� � (( <br />�(�tti l.a' Y <br />Title: <br />l�tA <br />V <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
