However, EHD has not received results from the <br /> secondary containment testing for the above referenced facility. On November 15, 2002, <br /> this facility was sent a certified letter stating that, "All results must be sent to this office <br /> within 30 days of the test completion. It is the responsibility of the owner/operator to have <br /> all test results submitted to this office no later than January 31,2003." (Attachment 1) <br /> On April 9, 2003, Michelle Le, Senior REHS, performed a routine underground storage <br /> tank inspection and requested the results from the secondary containment testing, the <br /> submittal of the certification of financial responsibility, and the letter from the chief <br /> financial officer (Attachment 2). On September 8, 2003, a follow up letter to the April <br /> inspection was mailed to the facility requesting the secondary containment test results and <br /> the submittal of the certification of financial responsibility and letter from the chief financial <br /> officer(Attachment 3). <br /> Additionally, on January 26, 1999,this department was notified by the City of Stockton Fire <br /> Department of unpermitted piping being installed at this facility by Elite IV Contractors. On <br /> January 28, 1999, Letitia Briggs, Senior REHS with EHD, issued the facility owner, Mr. <br /> Chris Buscaglia,a"Notice to Abate"stating that the newly installed piping must be exposed <br /> so that EHD can perform an inspection (Attachment 4). On May 6, 1999, a permit <br /> application was submitted to EHD by Elite IV Contractors for the installation of the product <br /> piping which had already occurred (Attachment 5).
