Michael Canlis Blvd. <br /> The permit to replace the monitoring system with a Veeder Root TLS-350 Plus console <br /> and Veeder Root sensors at the above referenced site has been approved with the <br /> following conditions. <br /> 1. Contact the local fire department and San Joaquin Valley Air District for any <br /> applicable permits. <br /> 2. At least one person who holds the International Code Council (ICC) UST Service <br /> Technician certification and manufacturers' certificate of training for all <br /> components installed or tested must be onsite when work is performed. <br /> 3. Each person installing or testing any component must hold a certificate of training <br /> from the manufacturer of that component. <br /> 4. All USTs must have the contents removed prior to beginning work. <br /> 5. EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring meets the requirements of California Code of Regulations Title 23 and <br /> the Health & Safety Code. Contact EHD for the following inspections (Allow a <br /> 48 hour notice for all inspections): <br /> a. Testing of all replaced sensors. <br /> 6. If concrete is broken near the underground storage tanks or over product lines <br /> they must be have an integrity test performed once concrete is poured in <br /> accordance to manufacturer guidelines. Results must be received by EHD within <br /> 7 days of returning the site to operating condition. <br /> 7. Update and submit a copy of the Monitoring and Response Plans and a UST <br /> Installation Certificate of Compliance (Former Form C) for each tank no later <br /> than 30 days after work is completed. <br /> 8.
