The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> a. Contact Michelle Henry to arrange the following inspections(a 48 hour <br /> notice is required): Verification of monitoring system to ensure it was <br /> unaffected by the EVR phase II upgrade. If necessary a functionality test must <br /> be performed to verify monitoring system meets the requirements of California <br /> Code of Regulations Title 23 and the Health& Safety Code. <br /> b. A copy of the alarm history prior to commencement of work and after <br /> completion of work must be available for review at the time of EHD inspection. <br /> 2. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site,at all times while work is being performed, <br /> who possesses all of the following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council (ICC),that are applicable to the work being performed,and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired, or serviced. <br /> And in addition <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 3. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. A valid,current California UST Service Technician certificate issued by the <br /> International Code Council(ICC), and <br /> c.
