If the soil removed during the sampling process is found to be contaminated it must be <br />disposed of at an approved disposal location. Treatment of soil onsite can only be performed <br />under a hazardous waste treatment permit. <br />2. A copy of the "Site Health & Safety Plan' must be on the job site for review. <br />3. See pages 7 & 8 of the permit application for required analytical tests and number of samples <br />to be taken and witnessed by this department. Form EH 23 046 Revised 08/01/11 must be <br />utilized. <br />4. In addition, the following information must be submitted within 30 days after the UST <br />removal date: <br />(a) Soil and piping rinsate analytical results must come directly to this department from a <br />third party California certified laboratory with affixed chain of custody. <br />(b) Hazardous Waste Manifest or hazardous waste determination for soil excavated during <br />the sampling process, piping removed and piping rinsate. <br />5. This pen -nit expires 180 days from the approval date. <br />6. Schedule the piping closure and sampling inspections at least 48 hours in advance with the <br />EHD inspector. The EHD inspector will witness the pipe grouting and the soil sampling. <br />7. Schedule secondary containment testing of the remaining product pipe and the tank top sump <br />within one week of returning the facility to operating condition. The EHD inspector will <br />witness the secondary containment testing.
