The Amended scope of work includes installing remote Annunciator, breaking ground to fix electrical conduits and install Icon electrical penetrations on the underground storage tank (UST) system at the above noted address. The permit has been approved with the following conditions: Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any applicable permits and requirements. 2. Any changes in information and scope of work must be submitted to EHD for approval prior to commencing work. All new addenda supersede the previous ones. 3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection appointments (a 48-hour notice is required for inspection appointments). Please be aware that inspections for emergency work must be scheduled within five days of permit approval. a. Sensor functionality: Functionality of the monitoring system must be verified to ensure that monitoring has been restored to meet the requirements of Title 23 California Code of Regulations and the Health & Safety Code. b. Overfill prevention verification: Verify that external audio and visual alarm activates when tank is 90% full. c. 24-hour hydrostatic test (or equivalent) on diesel piping sump after concrete is poured. Please be aware that the facility cannot operate the UST system or receive fuel in the diesel tank prior to the EHD inspection and approval. 4.
