The Amended scope of work includes installing remote Annunciator, <br />breaking ground to fix electrical conduits and install Icon electrical penetrations on the <br />underground storage tank (UST) system at the above noted address. The permit has been <br />approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. <br />b. Overfill prevention verification: Verify that external audio and visual alarm activates <br />when tank is 90% full. <br />c. 24-hour hydrostatic test (or equivalent) on diesel piping sump after concrete is <br />poured. <br />Please be aware that the facility cannot operate the UST system or receive fuel in <br />the diesel tank prior to the EHD inspection and approval. <br />4.
