Designated Operator, Philip Suah <br />(ICC #5249865 -UC, exp. 9/7/08). <br />• Updated UST Monitoring/Response Plan was submitted to you, via e-mail, for <br />your approval on 4/14/08. <br />Hag -Waste Inspection: <br />• #20 & #91 = waste drums on-site were picked up by Filter Recycling Service, <br />Inc. on 4/10/08 (see enclosed work record).
