The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br />applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Contact Cindy Vo at (209) 468-8257 to arrange the following inspection appointments (a 48-hour <br />notice is required for inspection appointments). <br />a. Witness soil sampling underneath the removed UDCs. <br />b. Line test on the 87, 91 and diesel primary pipe. Have a licensed tank tester conduct a 0.1 <br />gallon per hour test witnessed by EHD or the permit holder can program the monitoring panel <br />to run a 0.1 gallon per hour test and provide the results to EHD. <br />c. Secondary containment test on the UDCs and all product pipe, after backfill. <br />d. Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br />monitoring has been restored to meet the requirements of Title 23 California Code of <br />Regulations and the Health & Safety Code. If it is determined that a cold start is necessary, a <br />copy of the alarm history shall be printed prior to performing the "cold start" of the Veeder <br />Root system and made available at the time of EHD inspection. <br />e. Leak detector test: 87, 91, diesel leak detectors must be tested to confirm that the device can <br />detect a leak of 3 gallons per hour at 10 psi. <br />f. Verify product line cap at non -diesel dispensers. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Secondary containment testing for all product piping and UDCs are required six months after <br />installation. Provide 48 hours notification to the EHD when this test is scheduled. <br />5. Any individual performing the work of a service technician must meet the following qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />
