<br />I. SUMMARY
<br />On February 14, 2002, Michelle Le, REHS, performed an inspection on this facility. At the time of
<br />inspection, this facility was given an Environmental Health Department (EHD) report (Attachment
<br />1) stating they must perform a secondary containment test by December 31, 2002.
<br />On September 30, 2002, an EHD notification letter were mailed to the facility and owner stating
<br />secondary containment testing must be completed by January 1, 2003. (Attachment 2)
<br />On November 15, 2002, a certified EHD letter was mailed to the facility stating it was their final
<br />notification to perform secondary containment testing by January 1, 2003. (Attachment 3) In
<br />addition, numerous CUPA newsletters have been mailed to the facility stating the secondary
<br />containment test deadline. (Attachment 4)
<br />On January 9, 2003, Steven Shih, REHS, called Nancy Le (owner) at 462-7621 regarding the
<br />secondary containment test. Mrs. Le stated that Stockton Service Station did all the testing for this
<br />facility. Mr. Shih called Mark Handerson of Stockton Service Station to ask if the secondary
<br />containment test had been performed. Mr. Shih then called Mrs. Le back to inform her that the
<br />secondary containment test had not been performed.
<br />On January 14, 2003, Steven Shih, REHS, took a photo of this facility showing this facility is still in
<br />operation. (Attachment 5)
<br />On February 24, 2003, an EHD notification letter was mailed to the facility stating annual
<br />monitoring certification and line leak detector certification must be performed by March 25, 2003,
<br />and the results must be submitted to this office by April 25, 2003. (Attachment 6)
<br />On April 15, 2003, D&S Dragline, Inc. performed secondary containment testing on this site and
<br />discovered that the piping sump of the supreme tank failed (Attachment 7). On April 25, 2003, D&S
<br />Dragline, Inc. went back and passed this failed piping sump. (Attachment 8)
