Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br />Form C) for the diesel tank no later than 30 days after work is completed. <br />7. This permit expires 90 days from the approval date. <br />8. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
