A copy of the test report must be submitted within 30 days of the test. Immediately log into <br /> the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload a copy of the test <br /> report as a miscellaneous State-Required Document in the Underground Storage Tank section or schedule this test <br /> and provide 48 hours notification to the EHD. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. No maintenance and repair <br /> records were available for the L3 91 STP sump liquid sensor alarms on March 29 and 30, 2014. These records shall <br /> be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br /> sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br /> Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit <br /> copies to the EHD. <br /> This is a minor violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved, current copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to failed to check that all alarms from the attached alarm history on the April 2014 <br /> designated operator monthly inspection report were responded to appropriately. The missing alarms include: L3 91 <br /> STP sump liquid sensor alarms on March 29 and 30, 2014. According to the designated operator report, these were <br /> caused by a sensor malfunctioning, but he did not note how the site verified this or followed up to the alarm. During <br /> the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each <br /> alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report.
