Secondary containment testing was scheduled to be performed on October 14, 2014 and a test report has not been submitted to the CERS. A copy of the test report must be submitted within 30 days of the test. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload a copy of the test report as a miscellaneous State-Required Document in the Underground Storage Tank section or schedule this test and provide 48 hours notification to the EHD. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. No maintenance and repair records were available for the L3 91 STP sump liquid sensor alarms on March 29 and 30, 2014. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved, current copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to failed to check that all alarms from the attached alarm history on the April 2014 designated operator monthly inspection report were responded to appropriately. The missing alarms include: L3 91 STP sump liquid sensor alarms on March 29 and 30, 2014. According to the designated operator report, these were caused by a sensor malfunctioning, but he did not note how the site verified this or followed up to the alarm. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
