Main Street, Manteca. <br />Your application for permit to install new monitoring system at the above facility has <br />been approved with the following condition: <br />1. Contact Fire Department for applicable permit. <br />2. See page 2 of the application. <br />3. On-site audible and visual alarm is required. <br />4. Submit new monitoring and response plan to EHD prior to scheduling an inspection. <br />5. Annual line tightness test is required. Submit result to EHD annually. <br />6. Contact Michael Kith for inspection. All appointments must be scheduled 48 hours in <br />advance. <br />7. Any changes in the information provided in this application must be submitted to <br />EHD for approval prior to commencing work. <br />8.
