The 12/4/12 secondary containment test results indicate a failure at the 87-2 and 91 fill sump. L)n <br /> 1/2/13 1 called to verify status and I was informed that cont actor is waiting reply from Chevron to start <br /> the repair process. As of today a permit for the repair isrequested. All secondary containment for the <br /> UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair <br /> or replace the failed component under permit and inspection of the EHD. <br /> 310. Liquid was observed in all the fill sumps. If water could enter into the secondary containment by <br /> precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, <br /> make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br /> accordingly. Ensure that the all sumps are maintained free of liquid. The Technician removed the liquid <br /> and put it in the facility's hazardous waste drum. This is a repeat violation. <br /> Notes <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation to the EHD within 30 days of receiving <br /> this report. <br /> The designated operator was on the site today. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
