The monitoring response plans on site are current but not approved by the EHD. The monitoring <br /> and response plan must be current and approved by the EHD. <br /> • Per the comments noted in the inspection report,#5, sentences four and five: <br /> "A copy of the monitoring and response plan was reviewed and approved by the EHD. AN <br /> approved copy was put in file and onsite for your records". <br /> • 1 have also enclosed a copy of the current monitoring and response plan. <br /> ■ 23d. The designated operator employee training for Suliman Mangal (hired 3 months ago) was not <br /> performed and is not current for Natalie Miller. The designated operator shall train facility <br /> employees for which he or she is responsible in the proper operation and maintenance of the UST <br /> system once every 12 months and within 30 days of hire. The training shall include, but is not <br /> limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's monitoring best <br /> management practices <br /> 2. Employee's role with regards to monitoring equipment as specified in the facility's monitoring <br /> plan <br /> 3. Employee's role with regards to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD within 30 days of receiving this report. <br /> • The designated operator trained Suliman Mangal was on January 17, 2011 and Natalie <br /> Miller on January 20, 2011.
