Michael Canlis Blvd — SR0041993 — Exploratory permit and addendum. <br />The permit application to expose the return line for the exploration of a leak on generator and <br />addendum to repair the secondary piping of south diesel tank has been approved with the <br />following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control District for <br />applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to EHD prior to commencing <br />work. <br />3. All piping and connections shall be approved by an independent testing organization and <br />bear an approval label or marking (example: UL label). <br />4. At least one person who holds the International Code Council "UST Installer/Retrofitter" <br />certification must be onsite AT ALL TIMES when work is performed. <br />5. Contact Steven C. Shih at (209) 468-9850 to arrange the following inspection appointments; a 48- <br />hour notice for inspection appointments is required. <br />a. Piping test: pressure and soap test on secondary piping of supply line and return line of south <br />diesel tank. <br />b. Third party tank integrity test for south diesel tank and piping line tightness test for <br />supply and return line of south diesel tank must be performed once concrete is poured, <br />prior to the contractor signing off of the jobsite. The test results must be submitted to EHD <br />within 7 days of the test. <br />6. Permit expires 90 days from the approval date. Note: if an extension to the expiration date <br />is required, a written request must be received by this office prior to the expiration date. <br />If you have any questions, please contact Steven C.
