Michael Canlis Blvd — SR0041993 — Exploratory permit <br />The permit application to expose the return line for the exploration of a leak on generator has <br />been approved with the following conditions: <br />1) Provide permit addendum to this office once the causes of leak are identified. The addendum <br />shall include, but not limit to, the scope of work, proposed equipment specification sheet. <br />The permit addendum must be approved so that an inspection can be arranged and parts can <br />be approved prior to commencing work. Per Environmental Health assessment of findings, <br />new plans and a permit application may be required to be submitted. <br />2) Soil sampling may be necessary per Environmental Health assessment. <br />3) Permit expires within 90 days. Note: If an extension to the expiration date is required, a <br />request must be received in writing to this office prior to the expiration date. <br />If you have any questions, please contact Steven C.
