Wilson Way, Stockton <br /> The San Joaquin County Environmental Health Department(SJCEHD)has reviewed and <br /> approved the permit to replace the UDC 5/6 sensor at the above referenced location. The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br /> District for applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. All new addendums supersede the previous ones. <br /> 3. At least one person who holds the International Code Council(ICC)"UST Service <br /> Technician"certification and manufacturers' certificate of training for all components <br /> installed or tested must be onsite at all times when work is performed. <br /> 4. Each person installing or testing any component must hold a certificate of training from <br /> the manufacturer of that component. <br /> 5. The"Certificate of Compliance for Underground Storage Tank Installation Form C" shall <br /> be submitted by the owner or the owners' agent [pursuant to California Code of <br /> Regulations(CCR)2635 (d)] before this replacement will be considered complete by <br /> SJCEHD. <br /> 6. Contact Toua Yang at(209)468-3459 to arrange the following inspection appointment <br /> (s); a 48-hour notice for inspection appointment(s)is required. <br /> a. EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health& Safety Code. <br /> 7. Permit expires 90 days from the approval date.
