Wilson Way, Stockton,CA <br /> The San Joaquin County Environmental Health Department(SJCEHD)has reviewed and approved <br /> the permit to replace the premium unleaded(91)leak detector and two tank annular sensors at the <br /> above referenced location. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department,and the San Joaquin Valley Air Pollution Control District <br /> for applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. All new addendums supersede the previous ones. <br /> 3. At least one person who holds the International Code Council(ICC)"UST Service <br /> Technician"certification and manufacturers' certificate of training for all components <br /> installed or tested must be onsite at all times when work is performed. <br /> 4. Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 5. The"Certificate of Compliance for Underground Storage Tank Installation Form C"shall be <br /> submitted by the owner or the owners' agent [pursuant to California Code of Regulations <br /> (CCR)2635 (d)]before this replacement will be considered complete by SJCEHD. <br /> 6. Contact Toua Yang at(209)468-3459 to arrange the following inspection appointment(s); a <br /> 48-hour notice for inspection appointment(s)is required. <br /> a. EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health& Safety Code. <br /> b. Leak Detector Test: leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi. <br /> c. If fill spill buckets are removed at any time when work is performed,they must <br /> verify minimum 5-gallon capacity and pass a 60-minute lake test or equivalent. <br /> Proper drainage must also be verified. <br /> 7. Permit expires 90 days from the approval date.
