Contact the fire department and San Joaquin Valley Air District for any applicable <br /> permits. <br /> 2. Any changes shall be submitted to EHD for approval prior to commencing work. All new <br /> addenda supercede the previous ones. <br /> 3. All piping and components must bear a UL label. <br /> 4. At least one person who is certified, as an ICC UST Installer/Retrofitter must be onsite at <br /> all times when work is performed on any portion of the UST System. <br /> 5. Each person installing or testing any component must hold a certificate of training from <br /> the manufacturer of that component. <br /> 6. Contact EHD for all inspections(a 48-hour notice for inspection appointments is required). <br /> 7. Once concrete is poured all lines shall have an integrity test performed in accordance to <br /> manufacturer's guidelines. Results must be received by EHD within 7 days of returning <br /> the site to operating condition. <br /> 8. If concrete is broken near the underground storage tanks they must be have an integrity <br /> test performed once concrete is poured in accordance to manufacture guidelines. Results <br /> must be received by EHD within 7 days of returning the site to operating condition. <br /> 9. This permit expires on September 1,2006. <br /> 10.
