The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elianna Florido at (209) 468-0343 to arrange the following appointments (a 48-hour <br /> notice is required for inspection appointments). <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California Code <br /> of Regulations and the Health and Safety Code. A copy of the alarm history shall be <br /> printed prior to performing the "cold start" of the monitoring system and made available <br /> at the time of EHD inspection. <br /> b. Verification of conversion frames installation per the manufacturer's installation <br /> instructions (Bravo dispenser conversion evaluation form for each dispenser should be <br /> available onsite at the time of inspection). (Inspection completed 12/8/17) <br /> c. Verification of the dispenser footprints contained by the under dispenser containment <br /> sumps (UDCs) and verification that all potential product piping leaks will be captured in <br /> the UDC. (Inspection completed 12/8/17) <br /> d. Visual inspection of the piping within the dispensers for any product leaks. (Inspection <br /> completed 12/8/17) <br /> Please be aware that the facility cannot operate UST system prior to the EHD inspection <br /> and approval. <br /> 4. If concrete will be broken near a tank, a secondary containment test must be performed on the <br /> affected LISTS once concrete is poured in accordance with the manufacture's guidelines. <br /> 1868 E.
