Mr. von <br /> Abe called and left a message with EPIC to call him back, but no one called back. <br /> On November 21, 2008, permit application, SR0055979, was submitted by Stockton Service <br /> Station Equipment, Inc. to repair the secondary containment failures (Attachment 71). The scope <br /> of work submitted was insufficient and Mr. von FIbe called and asked for a detailed scope of work <br /> to be submitted. <br /> On December 3, 2008, Mr. Singh submitted a return to compliance certification, a corrective <br /> actions statement, and supporting documents in response to the October 22, 2008, routine UST <br /> and hazardous waste inspections (Attachment 72). Mr. von FICje spoke with Mr. Singh, who <br /> stated he is no longer using Stockton Service Station Equipment, Inc. to make repairs and has <br /> hired Elite IV Contractors, instead. <br /> On December 4, 2008, permit application, SR0056072, was submitted by Elite IV Contractors to <br /> repair the secondary containment failures (Attachment 73). The scope of work submitted was <br /> insufficient and Mr. von FlUe sent a deficiency letter on December 11, 2008, asking for a detailed <br /> scope of work to be submitted. <br /> On December 8, 2008, Mr. von FIbe spoke with Mr. Singh, who stated Stockton Service Station <br /> Equipment is in the process of making secondary containment repairs. Mr. von FlUe stated that <br /> neither permit was approved yet and suggested Mr. Singh call the contractor to find out what is <br /> happening. Mr. von FlUe later received a call from Mr. Randy Chadwick of Stockton Service <br /> Station Equipment stating he already made the repairs to the piping sump penetrations. Mr. von <br /> FlOe explained that he was still waiting for a scope of work and an equipment list and the permit <br /> was not approved yet. Mr. Chadwick promised that he will get the paperwork in and mentioned <br /> that he was only repairing the sumps and Elite IV was to repair the line. <br /> On December 10, 2008, Mr. von FlOe received a scope of work and equipment list from Stockton <br /> Service Station Equipment, approved the permit application, SR0055979, and witnessed the <br /> testing of the repairs on December 12, 2008 (Attachment 74). <br /> On December 12, 2008, a revised scope of work and equipment list was submitted by Elite IV. <br /> Manufacturer training for Ameron piping install was lacking. This was submitted later and Mr. von <br /> FICje approved the permit on December 15, 2008. <br /> On February 3, 2009, Mr. von FI(Je witnessed the testing of the diesel secondary piping, which <br /> passed (Attachment 75). <br /> 4747 West Lane, West Lane Chevron <br /> On April 9, 2004, Ms. Le sent letters dated April 5, 2004, to the owner, Mr. Surinder Singh Saini, <br /> and West Lane Chevron stating that the annual monitoring system certification and leak <br /> detector testing were past due (Attachment 76). <br /> On April 13, 2004, a permit application, SR0037599, was submitted to replace the spill <br /> containers, drop tubes, and mechanical overfill prevention devices as part of the Enhanced <br /> Vapor Recovery (EVR) upgrade.
