The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> a. Call Michelle Henry to arrange the following inspections (a 48 <br /> hour notice is required): Functionality of the monitoring system to <br /> ensure that monitoring has been restored to meet the requirements of <br /> California Code of Regulations Title 23 and the Health & Safety Code. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physicaiiypresentat the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced, and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 3. Permit expires 180 days from date of approval. Note: If an extension to the <br /> expiration date is required, a written request must be submitted to this office <br /> prior to the expiration date. <br /> Page 1 of 2 <br />
