The 12 East, 13 West, and 16 West float and chains in the truck -side under dispenser containments <br />failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be <br />maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it <br />detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor <br />replace the failed component with a functional component (LIS 113-x listed, if applicable) and obtain a <br />permit within one business day from the EHD. If the failed component can't be replaced immediately, <br />there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br />- Corrected on-site. Technician cleaned out the bravo float, re -test and was able to trigger shear valve. <br />18. Diesel West and Diesel East spill bucket failed. All spill containers shall have a minimum capacity of <br />five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />discontinue deposition of diesel into this tank until the component is repaired or replaced by a properly <br />licensed, trained, and certified contractor under permit and inspection of the EHD. If the spill container <br />can't be replaced immediately, there is a possibility that the diesel UST system may be red tagged to <br />prevent fuel input. <br />- Corrected onsite. Diesel West had the fill adapter tightened and Diesel East had its drain valve <br />replaced. Both passed the 1 hr hydrostatic test. <br />23c. No response action was noted for the alarms for L2 sensor out and fuel alarm on 09/10/10 and L16 <br />setup data warning on 08/24/10 and fuel alarm on 08/31/10. The Designated Operator noted "12 annular <br />and L16 have no work order to reference to at this time of inspection." No response action was noted for <br />L9 sensor out alarm on 07/15/10. The Designated Operator noted that "no documentation for sensor <br />out" for the L9. It was unclear whether a response action was taken for these alarms. Per Debbie, a call <br />was made to a service technician. If such action was taken, the action needs to be documented in the <br />monthly inspection reports. During the monthly inspection, the designated operator shall review the <br />alarm history for the previous month, check that each alarm was documented and responded to <br />appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report.
