The designated operator failed to visually inspect the Diesel truck side UDCs for leaks, water, or debris during tall of the past 12 monthly inspections. On some reports the UDCS are listed but not marked they have been checked. On others the UDCS are not notated at all.. During the monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the reports. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The (W) Bravo float and chain in UDC sump 15, (E) Bravo float and chain in UDC sump 13, (E) Bravo float and chain in UDC sump 12, (E) Bravo float and chain in UDC sump failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't be replaced immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. The floats were stuck at the time of inspection. This was corrected on site. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by May 29, 2015 . Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS. 69 I-r ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
