If the backfill material and soil is found to be contaminated it must be disposed of at an approved <br /> disposal location.Treatment of soil onsite can only be performed under a hazardous waste treatment <br /> permit. <br /> 2. A copy of the"Site Health&Safety Plan"must be on the jobsite for review. <br /> 3. See pages 7&8 of the permit application for required analytical tests and number of samples to be <br /> taken and witnessed by this department. An updated version of these pages,Revised 11/21/06,was <br /> inserted into the removal application. <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. EHD must witness: <br /> a. The one-hour or equivalent spill bucket test. <br /> b. Verify five-gallon capacity of the spill bucket. <br /> c. Drain valve operation or equivalent for the new waste oil spill bucket. <br /> d. Twenty-four hour or equivalent hydrostatic test of fill sump. <br /> e. Final inspection to ensure monitoring system operation. <br /> 6. The waste oil tank must have overfill prevention that satisfies CA Health and Safety Code Sections <br /> 25281.5 and 25291. Also CA Code of Regulations,Title 23,sections 2635 and 2636. <br /> 7. A"Certificate of Compliance for Underground Storage Tank Installation Form C" shall be submitted <br /> by the owner or the owners'agent[pursuant to(CCR)2635(d)]before this upgrade will be considered <br /> complete by EHD. <br /> 8. If concrete is broken near the underground storage tanks,the UST's must have an integrity test <br /> performed once concrete is poured in accordance with the manufacturer's guidelines.Results must be <br /> received by EHD within 7 days of returning the site to operating condition. <br /> 9. In addition,the following information must be submitted within 30 days of the removal date: <br /> (a) All analytical results must come directly from a third party California certified laboratory with <br /> affixed chain of custody or this office will not accept them. <br /> (b) Hazardous Waste Manifest for oily water and waste oil. <br /> 10. This permit expires on August 2,2007. <br /> 11. Arrange all inspections at least 48 hours in advance.
