No alarm logs were attached to <br />the designated operator inspection reports or available onsite for January 2014 through July 2014 operating months. <br />These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br />monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br />site. Submit proof of correction to the EH D. This is a repeat violation, Class II. <br />207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document the alarms from the attached alarm history on the February 20, 2014 <br />designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br />missing alarms include: L10 alarm on January 17, 2014. During the monthly inspection, the designated operator shall <br />review the alarm history for the previous month, check that each alarm was documented and responded to <br />appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly <br />report. Ensure that designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. This is a repeat violation, Class II. <br />The designated operator didn't appear to have verified that historical alarms were responded to appropriately. No alarm <br />logs were attached to the designated operator inspection reports or available onsite for the January 2014 through July <br />2014 operating months. These records shall be maintained on site for at least three years. During the monthly <br />inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was <br />documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br />response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. This is a repeat violation, Class II. <br />310 CCR 2636(f)(1) Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. <br />The 87 -octane fill sump sensor failed to activate an audible and visual alarm when tested to ensure continuous <br />monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. The technician onsite cleaned this sensor and retested <br />it with passing results. This is a Class II violation. This was corrected on site. <br />319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the 87 -octane STP sump, 91 -octane STP sump, and 91 -octane fill sump. If water could enter <br />into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The <br />service technician removed approximately 1/4 -gallon of liquid from the 87 STP sump, and approximately 2 cups of <br />liquid from each of the 91 STP and 91 fill sumps. Ensure that all sumps and annular spaces are maintained free of <br />liquid. This is also a repeat violation (previously cited as violation 310). This is a Class II violation. <br />325 CCR 2635(b) Spill container failed to contain a minimum capacity of five gallons. <br />The 91 -octane spill container failed when tested. All spill containers shall have a minimum capacity of five gallons and <br />be capable of containing a spill or overfill until it is detected or cleaned up. The technician onsite noted that the drain <br />valve was leaking. The drain valve was cleaned and the spill container retested with passing results. This is a Class II <br />violation.
