This information was required to <br />be submitted to the EHD by October 28, 2013; to date, no corrective action information was received by <br />the EHD. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br />billed at the current hourly rate ($125). Failure to submit the completed information immediately <br />may result in a re -inspection, additional violations, and/or further legal action. <br />The required corrective actions include: <br />Violation 104: Financial responsibility documents have not been submitted to the EHD, these documents <br />are required to be submitted annually. <br />Violation 105: UST's Facility Operating Permit Application is incomplete and UST Tank Information is not <br />uploaded into CERS <br />Violations 109, 110, and 111: Annual monitoring system certification, leak detector testing, and spill <br />container testing were performed on August 30, 2012 and was performed again on December 18, 2013 <br />and was past due. These tests are required every 12 months. <br />Immediately complete and submit a copy of the Return to Compliance Certification form with a statement <br />of how each violation was corrected, including supporting documentation, to the EHD.
