Monitoring system certification was canceled. Please be <br />aware that a re -inspection to observe the routine monitoring system certification will be required and will <br />be billed at the current hourly rate ($125 per hour). <br />Minor -Notice to Comply: <br />104. Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. A current copy of financial responsibility <br />was submitted to the EHD at the time of inspection; however, the submitted copy lacks a witness <br />signature. Immediately submit a completely filled out copy of the financial responsibility documents to <br />the EHD. Alternatively, log into CERS, upload the required information, and submit for review by the <br />EHD. Complete by October 28, 2013. <br />105. UST Facility Operating Permit Application is incomplete and UST Tank Information is not uploaded <br />into CERS. Immediately log into CERS and upload the required information for review by the EHD. <br />Complete a separate tank form for each tank. <br />Class II Violations: <br />109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on August 30, 2012 and was due by August 30, 2013. Testing has not been <br />completed and is past due. These tests are required once every 12 months. Immediately schedule <br />these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection <br />will be charged at our current hourly rate. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by October 28, 2013. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD 23-02 Rev 04/19/12 1 / Page 3 UST INSPECTION REPORT <br />
