Alarm log for L-1, 5 7/17/11 Sensor out and fuel alarm, L-4 7/17/11 High liquid alarm L-9 6/24/11 <br />Liquid warning alarm were not found on site. These records shall be kept on site for at least 3 years. <br />Immediately locate and maintain the alarm log on site and submit copies to the EHD within 30 days of <br />receiving this report <br />15. The November 25, 2008, secondary containment test results indicate a failure at the 87 fill sump. All <br />secondary containment for the UST system must be tight. Immediately have a properly licensed, <br />trained, and certified contractor repair or replace the failed component under permit and inspection of <br />the EHD. <br />17. The Diesel mechanical line leak detector failed to detect a leak when tested. All line leak detectors <br />shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak <br />detector, retested it, and it passed. <br />23c. The designated operator failed to document all the alarms from the attached alarm history on the <br />July, 2011 designated operator monthly inspection report and failed to check that they were responded <br />to appropriately. The missing alarms include L-9 premium turbine alarmed on June 24, 2011. During <br />the monthly inspection, the designated operator shall review the alarm history for the previous month, <br />check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br />history with documentation taken in response to any alarms to the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />24, 25, & 26. Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on June 23, 2010, and was due by June 23, 2011. Testing has not been completed <br />and is 2 months past due. These tests are required once every 12 months. Immediately schedule these <br />tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be <br />charged at our current hourly rate. Monitoring certification, spill bucket and Leak detector test was <br />performed today.
