Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility to the EHD within 30 days of receiving this report. 6. Approved copies of the monitoring and response plans were not found on site. Copies of these plans shall be retained on site at all times. Immediately locate and retain a copy on site. 7. Maintenance and monitoring records, were not found on site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the maintenance and monitoring records on site and submit copies to the EHD within 30 days of receiving this report. 15. The November 25, 2008, secondary containment test results indicate a failure at the 87 fill sump. All secondary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. 23c. The designated operator monthly inspection reports for the past 12 months were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD within 30 days of receiving this report. 24, 25, & 26. Annual monitoring system certification, leak detector testing, and spill container testing were last performed on June 23, 2010, and was due by June 23, 2011. Testing has not been completed and is 2 months past due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. Note Owner claims that all the paperwork for the facility was taken by the testing contractor and designated operator to be organized a couple of weeks ago. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, to the EHD within 30 days of receiving this report. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
