This <br />information was required to be submitted immediately. This information has not been received resulting in <br />continuation of non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($122). Failure to submit the completed information immediately may result in a re- <br />inspection or further legal action. <br />The missing information includes: <br />[-i Current UST Facility form (former Form A) ❑ Current UST Tank form (former Form B) <br />r <br />Repair and retest of 87 fill sump.
